Mostly documentation and test fixes without much effect on real code.

The biggest change is newly-connected sockets and opened files
(Kgio.tryopen) will have the close-on-exec descriptor flag set on it
when run under Ruby 2.0.0dev.  This is to remain consistent with the
behavior of core IO classes in Ruby 2.0.0dev.

accept()-ed sockets have always defaulted to close-on-exec in kgio,
regardless of Ruby version.

It's been a while since the last release, so I'm thinking about pushing
